Answer:
Stop in a safe location, check for injuries, call Police.
All crashes involving property damage or injuries should be reported to the police department of jurisdiction as soon as possible.
The Motorist Traffic Crash Report form contains information of all units and individuals involved in the accident, along with the report number, date and time of the accident.
Contact your insurance agent as soon as possible with information pertaining to the crash.
You will be given an orange copy of the Motorist Crash Report as your copy to submit to I.D.O.T. If damage to any vehicle in the accident exceeds $500.00 (estimated) or there is an injury, each driver is required by State Law to complete the Motorist Traffic Crash form and mail within 10 days to: Illinois Department of Transportation, 3215 Executive Park Drive, Springfield, IL 62756-0001. You may wish to maintain a copy of this report for your own records.
Police officers complete narratives and diagrams on injury accidents or accidents requiring the tow of one or more vehicles.
If a traffic citation is issued to the other driver you may not need to appear in court. However, if you do not appear in court and the other driver does, the court considers that since you did not appear as a witness to the accident, that the ticket may be dismissed. Always check with the court clerk on the day before a scheduled court appearance to see if a trial is likely to happen. motorist Traffic Crash Reports are available normally after three days of the accident and are available during normal business hours. A fee of $5.00 will be collected at the time of pick-up for a copy of this report.
